An unbackable favourite before the count, Cousins polled 20 votes to beat out fellow Eagles midfielder Daniel Kerr - who finished right on his heels with 19 - for the award.
With a Grand Final appearance looming, Cousins was not in Melbourne to receive the award and was instead presented with the medal by teammate Chris Judd at Burswood Casino in Perth.
Cousins' medal run started off shakily as he didn't poll his first votes until round four. He polled consistently after that, however, and received four best-on-grounds.
He received his last votes in round 18 and had to endure a nervous wait before being crowned the AFL's fairest and best player.
St Kilda midfielder Nick Dal Santo finished third in the count on 18 votes.
